Mechanical thrombectomy often requires several manoeuvres to reopen an occluded brain vessel. Complete reopening at the first attempt is called the first-pass effect.

The analysis of the international Stroke Thrombectomy and Aneurysm Registry (STAR), with Prof. Ilko Maier as first author, included 1,481 patients with anterior-circulation large vessel occlusion and complete reperfusion. Complete reperfusion with the first pass was an independent predictor of favourable functional outcome. First-pass success was more likely in M1 occlusions and in less severe strokes.

The findings underline the importance of fast and complete reperfusion in stroke care.
